Abstract
Background:
@EN Painful peripheral polyneuropathy is one of the most distressing complications of diabetes. The painful neuropathy often disrupts and reduces the overall quality of the patient's life. The clinical symptoms of peripheral polyneuropathy
include
severe dysesthenic during, most often in the feet and ankles and to a less extent in the upper extremities, with nocturnal worsening, thermal hyperalgesia, paresthesias, insomnia, anxiety, depression and the absenteeism from work.
Drug such as narcotic alageics, tricyclic antidepressants, atconvulsants ad phenothiazines have been used to treat painful neuropathy, but these drugs are not so effective for the many patents and the side effects may become intolerable.
Capsaincin (0.075%) has been used to treat postherpetic neuralgia and postmastectomy pain syndrome without the side effects associated with systemic drugs.
This study was conducted to estabilish the effects. of topically applied capsaicin ointment on the changes in symptoms in patients with painful diabetic neuropathy.
@ES Method:
@EN Subjects enrolled 13 patients, men (2) and women (11), with painful polyneuropathy. 0.075% capsaicin was applied to the painful areas 4 times/day for 14 days.
A visual analogue scale of subjective pain intensity through McGill pain qustainnare and objective basellinevibrating sense threshold cheked by Biothesiometer. After 2 weeks, test was repeated.
@ES Results:
@EN 1) baseline vibrating sense threshold in patient is more higher than controlled groups (p<0.01).
2) after treatment vibrating sense threshold in patients was not changed.
3) pain rating index decreased after 14 days topical capsaicin application.
@ES Conclusion :
@EN These result show that topical 0.075% capsaicin application is a useful for the pain control in painful diabetic polyneuropathy.
